modifier 25 is to be used with E/M codes when there is a CPT procedure that requires E/M evaluation as part of the CPT code.

Every CPT procedure has an E/M component like before suturing an open wound physician will requir to evaluate the wound wheather it will require suturing that is the reason modifier 25 should be used to identify seperately reportable E/M level with som CPT procedres
